This circuit could be employed when using three pickups, two volume controls (with push-pull switches) and a 3-way pickup selector. It is shown with, but not limited to a Gibson pickup selector. Switch positions and pickup assignments are mapped in the legend on the drawing. Enjoy!

Thanks for reposting this scheme. As JohnH pointed out in the other thread, one can use the P/Ps to "preset" selections, then use the toggle to move between them. Also, if one had a particularly mismatched pickup, a quick pull up and twist of the knob could alleviate the discrepancy when switching that pickup in/out.
While the truth chart is nice, it leaves the impression that we are merely reassigning volume controls rather than also switching the pups in/out. I thought your prose-based description from the other thread made it clearer, so I took the liberty of adding it here:
