mm,
This forum is in the midst of rescuing older images from Photobucket's recent user-hostile actions. The image is still visible in certain threads in these forums, but without knowing the thread titles where it still works, that's not going to be easy to search on.
This should fulfill your request.
What it does:
Pos 1 - Neck Pos 2 - Both in parallel Pos 3 - Both in series Pos 4 - Bridge
Note the "extra feature" of shorting the Neck pickup when it is not selected. Since the pup is always connected to the controls (and thus to the output line), and since it's not grounded in Pos 4, there's a slight chance of it picking up some hum, and sending it along to the output - it acts like an antenna. In theory, the extra wire short circuits the Neck pickup, which should reduce or eliminate hum while it's not selected.
