Strat mod: two volume, no tone, 5 way switch

Hi guys,

I am trying to mod my strat to the following:
- normal five way switch
- three pickups 
- two volume controls (one for the bridge, one for neck & middle, bridge pickup is a SD SH-4 JB with push-pull coil tap), no tone control
- one kill switch

I have a John Hewitt diagram which comes close but it does not work (perhaps I do something wrong, perhaps it is because the diagram also has a tone). 

Does anyone have any suggestions how I can wire my strat?

This makes it sound like you have already wired the guitar in some fashion, and now need to troubleshoot a wiring issue, rather than starting from scratch to wire the guitar with a new scheme. Do you need design help or troubleshooting help?

First, which of John's diagrams are you using? (I suspect his "Strat with 2 Volumes" design, but let's be clear).

Second, how is it wired at present? and, what about it doesn't work?
