I'm wiring and Ibanez GIO that has an HSS pickup configuration. The first position (humbucker) is very quiet and sounds kind of quacky. The second position works perfectly fine while the middle position is completely dead. The fourth and fifth positions work fine. I don't know too much about wiring so help is much appreciated.

It could be a lot of things. First off, what are you trying to do with this scheme? In other words, how is this guitar wired and what do you expect it to do? For example, what combines with what, are there coil splits, phasing or series\parallel modifications present...etc...?
Then there's the hardware. What pickups are you using, Are you using the original components or have you replaced the OEM stuff with aftermarket?
Finally, and this may not even be an issue, but did you test\verify that the components used are fully functional?

guitarwiringnoob- To add to what cynical1 wrote above, before anything else, verify which pickup coils are operating in which switch positions using the "screwdriver tap test" (or similar method). Again, this may depend upon the wiring scheme you are aiming for here, but with a standard Strat arrangement, it doesn't make much sense that position 3 (assuming it's supposed to be middle only) would be quiet while position 4 (N + M, presumably) "works fine". You might just be hearing the neck alone at that position. So, check to see which coils are operative in which switch positions, in addition to answering the things cynical1 asked.
