Phat Cat Wiring

I am trying to install two Seymour Duncan Phat Cat p-90's in a Strat one in the bridge and one in the neck position with a Lace Holy Grail in the middle position. I want to use a puch pull pot so that I can activate the two P-90's and not the middle pickup when needed. I also want to hook these up to a 5 way switch.
Anyone have any ideas or know where I might find a wiring diagram?

Just add the switch as a "neck-on" (or "bridge-on").
