As already written before, the decal was there when I bought the guitar. I've got no experience in creating decals, so I'll leave it as it is now.
There's a sound file with twelve tones, when the oop switching hadn't been installed yet.

I just played the same stuff 12 times according to the 12 switching positions the guitar had at that time, without changing anything else with the amp or the guitar. I used my Vox VT40+ with gain in mid position and a bit of reverb.
The twelve parts come up as follows:1. bridge
2. bridge and neck in parallel (the 3 original Tele sounds)
3. neck
4. middle (the added Fender pup)
5. bridge and middle in parallel
6. middle and neck in parallel
7. bridge, middle and neck in parallel
8. bridge in parallel with middle and neck in series
9. bridge and middle in series
10. bridge and neck in series
11. middle and neck in series
12. bridge, middle and neck in series
