It's come to my attention that this is the "Craig Anderton mod" and is not original to me. Here's a link to a previously documented version
This wiring arrangement uses a Strat's 5-way switch to add two extra pickup selections to a standard Telecaster. The extra pickup selections are in positions 1 & 2, leaving positions 3, 4 & 5 in "stock" order. I like this scheme because the Strat's 5-way switch is readily available, and is the only component that gets replaced on the Tele. Obviously you'll need to buy the cap as well (green circle in the diagram). Because this scheme has a series connection, if the Bridge pickup is "shielded," the shield wire must be separate from the negative wire.
